Unlock powerful data collection by creating public, shareable, and translatable web forms directly from your monday.com boards. Follow these simple steps to get started with TransForm.
1. Installation from monday.com Marketplace
Getting TransForm onto your boards is quick and easy:
- Navigate to the monday.com Apps Marketplace within your monday.com account.
- Search for "TransForm".
- Click "Use App" and follow the prompts. You'll be asked to choose which workspace and specific boards you want to add TransForm to.

2. Accessing and Configuring TransForm
Once installed, here’s how you set up your first form:
- Open TransForm View: Go to a monday.com board where you've installed TransForm. Open the "Board Views" menu (usually found at the top of your board, it might look like an eye icon or say "+ Add View"). Select "TransForm" from the list of available views to open the app interface.

Select Columns for Your Form: In the TransForm interface (toolbar on the left), you'll see a list of columns from your current board. Check the boxes next to the columns you want to include as fields in your public web form. The form preview on the right will update as you make selections.
Note: People columns and Subitem columns are typically excluded from form generation and won't appear in the selection list.

Customize the Item Name Field (Optional):
TransForm will usually use your board's "Name" column (or the first column if no "Name" type column exists) for the main identifier in the form preview. You can customize the display title for this field:
- In the form preview area, locate the label for this "name" field.
- Click on the label (or the pencil icon next to it if visible).
- An input box will appear. Type your desired custom title (e.g., "Project Name," "Client Inquiry," "Your Full Name") and press Enter or click away to save.

3. Sharing Your Form
With your form configured, it's time to share it:
- Generate Share Link: Once you're happy with the selected fields, click the "Share form" button (usually at the top right of the TransForm app interface).
- Copy Your Form URL: A popup will appear. If this is the first time sharing for this board and configuration, TransForm will generate a unique URL. This URL will be displayed in an input field.
- Copy and Distribute: Click the "Copy link" button next to the URL. Share this copied URL anywhere you need to collect data – email signatures, social media posts, your website, direct messages, etc.

4. How Respondents Use the Form
Here's what your audience will experience:
- Accessing the Form: Anyone with the link can open the form in their web browser.
- Language Selection: The form page includes a language selector (usually a dropdown). Respondents can choose their preferred language from a comprehensive list, and the form's title, field labels, and options (for status/dropdown) will instantly translate.
- Filling Out and Submitting: They fill out the fields as per your configuration (text, numbers, dates, dropdowns, etc.) and click the submit button.
- Confirmation: After successful submission, they'll typically see a "Thank You!" message or a similar confirmation on the page.

5. Managing Form Access & Submissions
Control your forms and see the data flow into monday.com:
- Activating/Deactivating Forms: Re-open the "Share form" popup within TransForm (on your monday.com board). Here, you can toggle your form's access between "Active" and "Inactive". Deactivating a form prevents new submissions and shows an "inactive" message to anyone trying to access it with the link.
- Automatic Item Creation: When an active form is submitted, TransForm automatically creates a new item in the monday.com board from which the form was generated. The data entered into the form fields will populate the corresponding columns in the new item.
- Data Re-translation (if applicable): If a form was submitted in a language different from your board's original language (which TransForm attempts to detect during the initial form configuration), the submitted text data will be automatically re-translated back to the board's original language before being saved in the item. This ensures data consistency for your team.
6. Tips for Best Results
- Clear Column Titles in Monday.com: The titles of your monday.com columns are used as the default labels on your form. Make them clear and user-friendly. Remember you can customize the main "Item Name" field's display title directly in TransForm.
- Choose Appropriate Column Types: The column types on your monday.com board (e.g., Text, Numbers, Date, Status, Dropdown) directly influence the type of input field rendered on your public form. Plan your board structure accordingly.
- Keep it Concise: While you can include many fields, overly long forms can deter submissions. Only include essential fields.
- Test Your Form: Before sharing widely, open the form URL yourself (perhaps in an incognito window) and submit a test entry. Check that it works as expected, the data appears correctly in your monday.com board, and try out the language selector.
- Inform Respondents (Optional): You might want to add a note on your website or wherever you share the link, briefly explaining that the form is multilingual.
